Please use this form to make payments to the Federal Judiciary. The type of payments you can make using this form include but are not limited to: contributions to Judicial Officer Retirement Fund (JORF) and Judicial Survivors Annuities System (JSAS), uncollected FEGLI and FEHB premiums while on Leave Without Pay status, improper payments of salary, bonus, or allowance, jury duty refunds, military service deposits, NSF fee for returned checks, overpayments of leave and travel or relocation reimbursements, pecuniary liability for improper payments made by disbursing and certifying officers, reimbursements for damaged/stolen/unreturned Judiciary property or equipment, reimbursements for non-compliance with training or tuition agreement, transit subsidy refunds, uncollected Social Security and/or Medicare tax for relocation and unsettled travel advance or interim salary payments.
