This form is only to be used when funds need to be established with Private Parties. It is not to be used by customers who owe PSNS&IMF funds due to overpayment on a contract.

Prior to submitting a payment to PSNS & IMF using this form, you should already have established communications with the appropriate PSNS & IMF contact. If we are unable to identify what the payment is for or if you do not list a PSNS & IMF point of contact at the time of payment, it may be returned.
