Only Case Trustees may use this form to deposit Unclaimed Funds or Undistributed Small Dividends. Only ACH deposits accepted.

This Pay.gov form may be used only by the Office of the United States Trustee or a bankruptcy case trustee to deposit:

• Unclaimed funds pursuant to 11 U.S.C. 347, or
• Undistributed small dividends pursuant to Federal Rule of Bankruptcy Procedure 3010.

THIS FORM IS FOR USE ONLY IN CASES FILED IN THE U.S. BANKRUPTCY COURT FOR THE DISTRICT OF CONNECTICUT.

• Do not use this form to make payments to the Court (e.g., filing fees or other court fees).
• This form is for depositing funds with the Court only.
