Use this form to pay filing fees and make installment payments for Chapter 7, 11, and 13 filing fees for the U.S. Bankruptcy Court for the Eastern District of Wisconsin. If this is for a new case and you have not yet received a case number, enter New Case in the case number field.

This form is only for U.S. Bankruptcy Court for the Eastern District of Wisconsin.

*Do not use this form for your Chapter 13 plan payments to the trustee or the Chapter 11 quarterly fees to the trustee.
