This site will process grant funds returns to the U.S. Department of the Treasury for the Homeowner Assistance Fund (HAF) Program that were unable to be obligated and expensed by the end of the program.

Homeowner Assistance Fund (HAF) recipients use this form to return to U.S. Department of the Treasury (Treasury) HAF funds that were unable to be obligated by 9/30/2026, or in the case of early closeout, by the date the recipient submits its final reports to Treasury, and pay other amounts related to award closeout. This form can also be used when a recipient intends to withdraw its participation in the program and return its full award. 

To successfully complete all the required fields in the form, make sure to use the notice received from Treasury, which includes the full amount to return, for the Recipient information section. If needed, the ACH Company ID is 2001000112.
