Emergency Rental Assistance Program (ERA1 and ERA2) grantees use this form to return funds that were unable to be obligated and expensed by the end of the Period of Performance and pay other amounts related to award closeout to the U.S. Department of the Treasury (Treasury).

You may also need to contact your financial institution beforehand if you have an ACH Debit Block, ACH Positive Pay, or ACH Fraud Prevention Filters on your bank account to prevent unauthorized ACH Debit transactions. If so, please authorize Department of the Treasury (Company ID 2001000108) to complete an ACH Debit from your bank account for this return of funds.
