Emergency Rental Assistance (ERA1) Program grantees use this form to return funds that were unable to be obligated and expensed by the end of the Period of Performance and pay other amounts related to award closeout to the U.S. Department of the Treasury (Treasury).

To successfully complete all the required fields in the form, make sure to use the notice received from Treasury, which includes the full amount to return, for the Grantee information section. If needed, the ACH Company ID is 2001000108.
