Use this form to apply and make payment for a Special Use Event Permit, Shoreline Use Permit or Vegetation Modification Permit for Berlin Lake.

NOTE: You will be able to upload a PDF attachment with dock drawings and pictures.  You can only attach one file and the max size is 15 MB.  Please prepare your PDF file before completing this form.  If you are unable to create a PDF file or your file exceeds 15 MB, please email this documentation to our office.
