Instructions for TTB Report of Wine Premises Operations
GENERAL INSTRUCTIONS:
Who prepares this report?
If you are a bonded winery or bonded wine cellar, you must prepare and file this report about your TTB permit operations.
When must I file this report?
The Report of Wine Premises Operations is due 15 days after the end of the period that the report covers. Print a copy for your records.
How do I file this form?
- Log onto Pay.gov using the ID and password that you were provided after registration.
- You will be advanced to the Pay.gov Main Menu screen. Select “List All Forms” by clicking the hyperlink. Then select the Monthly Report – Report of Wine Premises Operations Form 5120.17 by clicking on the form number that is listed as a hyperlink.
- After completing the electronic monthly report, simply click “Submit” to file the report electronically.

What is my employer identification number?
This is a nine-digit code that the Internal Revenue Service (IRS) has assigned to your business.
What is my TTB (Tax and Trade Bureau) registry number?
Each location at which you conduct operations has a registry number. TTB assigns this number on your permit as a producer of wine. The format should be BW-XX-9999, BWC-XX-9999(9) or BWN-XX-99999 where XX is a 2-letter state abbreviation. If you have more than one location, file separate reports for each. You cannot enter any extra spaces or leading zeros within the Registry Number, as this will cause an error on your form.
Who may file this report?
The person who has been authorized by TTB to use the Pay.gov system for the permitee.
