GENERAL INSTRUCTIONS:

When must I file this report?
How do I file this form?
  1. Log onto Pay.gov using the ID and password that you were provided after registration.
  2. You will be advanced to the Pay.gov Main Menu screen. Select “List All Forms” by clicking the hyperlink. Then select the Monthly Report – Report of Wine Premises Operations Form 5120.17 by clicking on the form number that is listed as a hyperlink.
  3. After completing the electronic monthly report, simply click “Submit” to file the report electronically.

INSTRUCTIONS FOR ITEMS ON THE FORM

What period(s) must my report cover?

Annual filers (20,000 gallons or less of inventory
and must be eligible to file annual tax returns)
Reports for: Period of time:
Starting business From the day and month that you started business to the end of the year.
Discontinuing or transferring business From the first of January to the end of the month that you discontinue or transfer your business.

Monthly filers (Inventory of more than 20,000 gallons)
Reports for: Period of time:
Starting business From the day and month that you started business to the end of the same month.
Discontinuing or transferring business From the first day of the month to the day of the same month that you discontinue or transfer your business.
